Rodent History Questionnaire
Receipt of rodents from non-approved sources must be approved in advance by the Attending Veterinarian or his/her designee. The Rodent History Questionnaire and the Rodent Quarantine Procedures forms, along with the Non Agricultural Animals Requisition forms must be completed to initiate importation of animals for non-approved vendor sources. The facility manager along with the principal investigator should complete appropriate sections of the Rodent History Questionnaire and the Rodent Quarantine Procedures forms. Completed forms should be faxed to 4-5649.
