OLAC Clinical Incident Report
For Research and Teaching Animals

This report MUST be completed and faxed (865.974.5649) or emailed (olac@utk.edu) to the OLAC Veterinarian (Dr. Patricia Coan) within 72 hours each time new diagnostic +/or therapeutic measures are begun for a research or teaching animal. This document meets 2.33(a)(3)(b)(3) of the Animal Welfare Act and Regulations as a mechanism of direct and frequent communication so that timely and accurate information on problems of animal health, behavior, and well-being is conveyed to Dr. Coan.
